We are a team of pharmacists and nurses providing consulting for long term care facilities, assisted living facilities, residential homes, LTAC units, dialysis centers, subacute and transitional care facilities. The CRS team is comprised of consultants who are Board Certified Geriatric Pharmacists accredited by the American Society of Consultant Pharmacists, Certified Consultant Pharmacists by NJCCP, and have received training in Antibiotic Stewardship by the Society of Infectious Disease Pharmacists.
Our preventative efforts represent significant savings to the health care facilities we serve; hospitalizations are averted, and medication administration time is decreased. CRS professionals conduct medication regimen reviews to assess the resident’s clinical therapies. Our comprehensive review assesses medication effect in the management of resident disease states. Dosage, renal status, medication allergies, concomitant disease states, and laboratory results are evaluated to ensure the desired outcome is achieved and adverse effects are prevented. We attend QAPI, antibiotic stewardship meetings, infection control meetings, and psychoactive utilization reviews.
